Position Summary: As a Registered Nurse working under the direction of the Director of Nursing, this position is responsible for the development, coordination, and completion of MDS assessments for Weyrich Health Care Center, a skilled nursing facility.


Essential Job Functions:

  • Conducts and coordinates the development and completion of preliminary and comprehensive resident assessments.
  • Ensures important resources are made available to patients and that patient care is delivered effectively and satisfactorily in accordance with Weyrich HCC standards.
  • Develops and updates a comprehensive resident assessment and care plan for each resident.
  • Evaluates each resident’s condition and pertinent medical data to determine any need to amend the admission assessment.
  • Ensures that care plans include measurable objectives and timetables to meet the resident’s medical, nursing, mental and psychosocial needs.
  • Start Medicare coverage for newly qualified patients or send out denial letters.

Working Conditions:

  • Works in well lighted/well ventilated area of the Weyrich Health Care Center
  • Able to sit, stand, bend, lift and move intermittently during working hours
  • Subject to frequent interruption
  • Be willing to work beyond normal working hours, and in other positions,
  • temporarily, when necessary
  • Attend and participate in continuing educational programs
  • Exposure to body fluids and body waste

  • Rotation of work hours



  • Maintains an active, unrestricted Registered Nurse license in the state of Arizona


Special Requirements:

  • Must be able to work weekends or a flexible work schedule.
  • Must be able to work some holidays.
  • Must be flexible to work another shift, if necessary.
  • Must have a high school diploma or equivalent.
  • Two years previous experience in a long term care setting is preferred.
  • Two years MDS skilled care is preferred.
  • Must have minimum four years’ experience as a nurse.
  • Must be able to work well with a team.
  • Working knowledge of nursing techniques, hygiene and safety precautions in a residential environment.
  • Must be able to effectively communicate in English.
  • Must be able to continuously stand and walk, squat, bend and lift 50 pounds.
