Job Description
As a Specialty Site of Care Coordinator, you will support the Coram Specialty Patient Experience Center located in the Mount Prospect Branch by serving as a single point of contact for an enhanced patient care model. As a Coordinator you will be responsible for serving as the liaison between our key customers accounts and patients and will coordinate with our key internal partners to ensure patients are on service timely. The coordinator will provide monitoring, supporting, and the processing of new patient referrals in this program and ensure we have all required information to successfully onboard our specialty patients.

To be successful in this role you will be responsible for ensuring initial patient outreach, understand patient site of care needs, and collaborate with Coram’s admissions, operations, patient’s advocacy, and payer account manager team to ensure timely patient onboarding. You will be responsible for the resolution of patient’s inquiries by identifying problems and coordinating appropriate corrective actions. You will work to resolve any escalated issues with patients and customer accounts and represent the Coram business and core values.

As a Specialty Site of Care Coordinator you will:
* Collaborate with key cross functional internal and external business partners to onboard patients to service meeting key SLA’s
* Organizes and maintains records and develops various reports to provide updates to assigned customer account case manager at established times.
* Participates in conference calls with customer account managers and internal partners while maintaining organized work flow and prioritizing multiple tasks.
* Develops and maintains patient relationships while providing responsive service to assigned patient caseload.
* Other duties as assigned

Required Qualifications
* Minimum three years of experience in patient care/customer relations, or specialty pharmacy.
* Excellent communication, organizational, customer service, and interpersonal skills.
* Experience detail orientation and time management skills.
* Experience working in Microsoft Office, specifically Excel, Outlook and Word
* Ability to travel as necessary (less than 10%).

Preferred Qualifications
* Education or experience equivalent to an Associate’s degree.
* Three to five years of experience in patient care/customer relations, or specialty pharmacy.

Education
Verifiable High School Diploma or GED is required.
