Location: Las Vegas, NV
The Community Services Care Coordinator partners with children and families in our community helping to manage and coordinate medical and mental health care and other services and supports. Care Coordinators monitor and coordinate client care coordination/treatment plans, educate them about their condition(s), connect them with service providers, and evaluate their progress.
This position helps clients by identifying service needs and assisting with the utilization of service providers and community resources that strengthen and enhance the quality of their lives.
- Bilingual, English and Spanish, preferable;
- Bachelor’s degree in behavioral health or social service-related field, required and two (2) years behavioral health or social service work experience; or
- Bachelor’s degree in non-behavioral health or social service-related field and four (4) years behavioral health or social service work experience;
- Fingerprint Clearance Card;
- Pass Centralized Screening;
- Ability to travel when needed; and
- Supervisory or management experience preferred.
JOB RESPONSIBILITIES AND COMPETENCIES
Provide general case management (including intake) and referral services to all clients needing such assistance.
- Receive and respond to phone calls and inquiries from primary medical and mental health professionals and clients.
- Partner with a client’s primary medical and mental health professionals to develop, implement, and monitor a care coordination/treatment plan.
- Coordinate and schedule consultations and appointments with primary medical and mental health professionals in collaboration with and/or on behalf of clients.
- Establish linkages with CPLCNV entities and outside service providers in the community.
- Educate clients on services/resources, application procedures and client rights, and reporting processes and procedures etc.
- Record pertinent data and information into an Electronic Health Record.
- Manage support services where appropriate.
- Help build informal support networks for a holistic person-centered approach.
- Educate CPLCNV staff and community professionals on issues related to care coordination and community service coordination.
- Participate in on-going training programs.
- Other job-related tasks as assigned.
Interpersonal skills that allow for professional and positive relationships with medical and mental health professionals, co-workers, clients, caregivers, and other community members are necessary. Good communication skills are evidenced by a culturally sensitive approach, effective listening, and positive interactions. Exceptional customer service skills are required. Ability to present operational information to personnel at all levels is also required. Must be able to travel, if needed.
Employee will be expected to participate in continuous learning, competency building, and maintenance of competency skills.
TYPICAL WORKING CONDITIONS
Work is performed in structured offices, community service centers, and/or medical/mental health clinic settings.
Employee is also accountable for understanding and complying with CPLCNV policies and procedures.
